Dear Residents and Families,

With the rise of COVID-19 in our community, We’ve had 1 staff member and 3 residents test positive. The residents and staff are doing well, and we are following CDC and NJDOH guidelines regarding quarantine and isolation precautions. We will be testing the residents on that unit and the unit where the staff member and residents were positive.

All residents that were seen by the vendor have been tested and their families have been notified. All residents with potential exposure remain negative. Hello residents, families, and friends of residents – there was a positive staff member that may have potentially exposed our residents to COVID last Thursday night on our West Wing unit. The staff person tested positive on Monday and notified the facility today. All the residents were rapid tested and results were negative. We would like to take this opportunity to notify families that our last outbreak is officially concluded. We have not had a positive within our building for the last 28 days. December 29, 2021

Salem County’s transmission rate remains at a HIGH level and un-vaccinated staff must be tested 2x per week. Vaccinated residents and patients do not need to be routinely tested according to these guidelines.

On Monday, lab testing was completed with staff for COVID and we have confirmed positive staff. In addition, one resident tested positive via a rapid antigen test this morning. We are currently in an outbreak. Testing will continue for all staff and residents twice weekly for 28 days until there are no longer any confirmed positives.

All newly admitted residents and patients that are not vaccinated will continue to be tested in accordance with our testing protocols, and we will also continue to test residents and staff if they display any COVID-like symptoms.

Indoor visitation continues, but we ask that you call prior to your visit and limit your visits to designated areas. Outdoor visitation for any resident not on our observation hall, as well as Skype and Facetime, calls will continue.

Hello, Friends and Families of Eagleview. We would like to take this opportunity to update you on visitation guidelines within our facility while the holidays are approaching. Visitations with families and friends are open to our residents, however, there are certain guidelines and restrictions that need to be maintained during your visit for safety purposes. Please read below for the guidelines. You can call the facility’s main number or our COVID hotline number (609-202-1243) for any further questions.

Thank you and have a wonderful holiday.

  1. Notification is preferred an hour prior to visitation in order to have your family member ready and to respect your family member’s roommate and their space.
  2. The resident (or authorized representative) and visitor must sign a risk acknowledgment form prior to visitation(s).
  3. Only two visitors per visit.
  4. Visitor temperature and COVID screening form must be obtained and filled out prior to visitation by ALL visitors.
  5. The resident and visitor must always wear source control (i.e. a mask or face covering) during the visitation.
  6. Maintain a distance of 6 feet between the visitor(s) and the resident during your visit.
  7. Remain in the designated area during your visit.
  8. Food is NOT permitted during visits. You may bring labeled, packaged items for the resident.
  9. The visitor(s) will NOT be able to use the facility’s restrooms during their visit.
  10. If you feel you have any COVID symptoms up to 14 days after your visit, please notify the facility immediately for contact tracing to begin.
  11. If you have cold-like symptoms on the day of your planned visit, please consider the safety of your family member and other residents and consider visiting on a different day.

Salem County’s transmission rate remains at a HIGH level (RED).

We continue to remain vigilant during our outbreak. Unfortunately, a resident has tested positive after being sent to the hospital. This restarts our outbreak status on 11/09/2021 and we will continue with routine COVID testing of staff and residents twice weekly for 28 days.

All newly admitted residents and patients that are not vaccinated will continue to be tested in accordance with our testing protocols.

Indoor visitation remains on hold pending the results of our facility-wide testing for 28 days from 11/09/2021. Scheduled outdoor visitations will continue for any resident not on our observation hall. Scheduled window visits, as well as Skype and Facetime calls can continue for all residents.

We continue to offer COVID Vaccines to residents and staff through our partnerships with our Local Department of Health, our pharmacy, and Inspira Hospital. In-between onsite vaccination clinics we will assist our staff to schedule appointments to Walmart, Walgreens, CVS, or Inspira to receive vaccinations.

October 31, 2021

Salem County’s transmission rate remains at a HIGH level and un-vaccinated staff must be tested 2x per week. Vaccinated residents and patients do not need to be routinely tested according to these guidelines.

That being said, on Thursday, 10/25/2021 and 10/28/2021 we tested all of our residents and one of our staff and one of our residents that were tested on 10/28/2021 came back positive. According to CDC guidelines we we will need to restart our 28 day outbreak testing of all staff and residents regardless of their vaccination status 2 x per week. We rapid tested staff and residents on 10/31/2021 and will continue testing staff and residents on 11/1/2021. All rapid tests have returned negative.

All newly admitted residents and patients that are not vaccinated will continue be tested in accordance with our testing protocols, and we will also continue to test residents and staff if they display any COVID-like symptoms.

Indoor visitation remains on hold pending the results of our facility wide testing for 28 days from 10/28/2021. Outdoor visitation for any resident not on our observation hall, as well as Skype and Facetime calls will continue.

That being said, on Friday, 10/8/2021 we rapid tested all of our residents and one of our residents tested positive. We conducted a 48 hour tracer and anyone coming in contact with this resident was called and told to be tested. According to CDC guidelines we restarted our 28 day outbreak testing of all staff and residents regardless of their vaccination status 2 x per week. As of today, Friday 10/15/2021 all other resident and staff tests have returned negative. Our Staff member that tested positive is now resolved and back to work and our resident that tested positive is in the hospital.

All newly admitted residents and patients that are not vaccinated will continue be tested in accordance with our testing protocols, and we will also continue to test residents and staff if they display any COVID-like symptoms.

Indoor visitation remains on hold pending the results of our facility-wide testing for 28 days. Outdoor visitation for any resident not on our observation hall, as well as Skype and Facetime calls will continue.

The CDC changed the way we look at COVID in our communities. We are no longer looking at our County’s positivity rate. We are now looking at our County’s transmission rate, and according to Salem County’s transmission rate, we are registering at a HIGH level and un-vaccinated staff must be tested 2x per week. Vaccinated residents and patients do not need to be routinely tested at this time according to these guidelines.

That being said, on Thursday 10/7/2021 we tested all of our staff and they were all negative. On Friday, 10/8/2021 we rapid tested all of our residents and one of our residents tested positive. We conducted a 48 hour tracer and anyone coming in contact with this resident was called and told to be tested. According to CDC guidelines we must restart our 28 day outbreak testing of all staff and residents regardless of their vaccination status 2 x per week.

Our newly admitted residents and patients that are not vaccinated will continue be tested in accordance with our testing protocols, and we will also continue to test residents and staff if they display any COVID-like symptoms.

Last week I reported that the CDC changed the way we look at COVID in our communities. We are no longer looking at our County’s positivity rate. We are now looking at our County’s transmission rate, and according to Salem County’s transmission rate, we are registering at a HIGH level and un-vaccinated staff must be tested 2x per week. Residents and patients do not need to be routinely tested at this time according to these guidelines.

That being said, last Thursday, 9/302021 we tested our unvaccinated staff and one of our non-direct care/non-nursing staff tested positive. The results were received late Friday night. We conducted a 48 hour tracer and anyone coming in contact with this staff member was called and told to come in to be tested. According to CDC guidelines we will now be conduct testing of all staff and residents regardless of their vaccination status every 5 days starting Saturday 10/2/2021 for a period of 14 days. Our newly admitted residents and patients that are not vaccinated will continue be tested in accordance with our testing protocols, and we will also continue to test residents and staff if they display any COVID-like symptoms.

Salem County’s COVID Scores are still at a MODERATE level, as well as every County in New Jersey. We did have one of our staff test positive on 7/27/2021. We trace tested staff and residents that might have come in contact with this employee and all tested negative. We then tested all of our residents 2x and they tested negative. We tested staff 2x and they also tested negative.

Our positive case is now considered resolved by our local DOH.

Staff that are not vaccinated will now be tested weekly and results will be posted as received. Residents do not need to be routinely tested.

New admitted residents that are not vaccinated will continue be tested in accordance with our testing protocols. We will also test residents and staff if they have symptoms.

Salem County’s COVID Scores are at a MODERATE level, as well as every County in New Jersey. That being said, we did have one of our staff test positive on 7/27/2021. We trace tested staff and residents that might have come in contact with this employee and all tested negative. We then tested all of our residents and they tested negative. We tested staff and are waiting for results.

Per CDC and DOH guidance, we will be testing both staff and residents again next week. Results of staff and resident testing will be posted as they are received.

Routine testing of our employees was completed on 05/12/2021. There were 2 new employees identified with a positive COVID-19 result. These employees have been notified and are quarantining at home. Today, we conducted our first series of COVID-19 vaccinations for staff and residents. The vaccines were administered in partnership with CVS pharmacy. Monitoring for any adverse reactions is ongoing, but at the time of this posting, all staff and residents are tolerating the vaccine well. Our next vaccination date is scheduled for February 2, 2021.

We have conducted additional facility-wide rapid testing of staff members. We detected 3 staff members positive through rapid testing procedures. The staff members have been promptly restricted from work. Facility operations and staffing remain stable and in good order. We have received notice from our laboratory of 1 resident with a positive COVID-19 result. The positive resident has been promptly moved to our isolation wing for close monitoring and treatment in a separate area of our building. The resident is receiving appropriate medical care under the direction of our medical director. Facility operations and staffing have not been adversely affected.

December 17, 2020

This past Wednesday, December 16th EAGLEVIEW conducted facility-wide testing of our residents and staff as per the NJ-DOH guidelines. We have received notice from our laboratory of two residents testing positive and 2 staff members tested positive. (One staff member has been on quarantine since 11/30 and due to exposure in the community and was tested before returning to work. This employee did not return work before testing positive).

The positive residents have been promptly moved to our isolation wing for close monitoring and treatment in a separate area of our building. They are receiving appropriate medical care under the direction of our medical director. Normal operations have not been adversely affected and our staffing is fine to meet all required care.

We will continue with weekly surveillance testing and rapid testing as needed per the NJDPH, CMS, and local guidelines.

Resident safety is our top priority. We are doing everything we can to ensure we prevent the possibility of the spread of COVID-19 within our facility, including staying in close communication with local and state health officials to ensure we are taking all the appropriate steps.

Per the State of New Jersey Department of Health Executive Directive No. 20-026, Eagleview Healthcare and Rehabilitation will be allowing indoor visitations. Indoor visitations will be held inside in the front lobby of the building from 1 PM  5 PM on prearranged scheduled days beginning November 12, 2020. 

Prior to your indoor visitation, an informed consent form must be filled out by both the visitor and resident (or authorized agent) and an agreement to follow guidelines for indoor visitation. Rules for engagement include: 

  1. An appointment must be made prior to visitation.No appointment = no visit. Outdoor visitations are always preferred. Please contact the receptionist at 856-451-5000 to schedule a day and time.  
  2. Visits may last for a 30-minute duration. Please arrive 15 minutes prior to your visit appointment time to complete pre-screening.
  3. Understand that visitations could be canceled if circumstances do not permit visits that day.
  4. The resident (or authorized representative) and visitor must sign an informed consent form prior to visitation(s).
  5. Only one visitor per visit for indoor visitations. No visitor younger than 18 years of age.
  6. Visitor temperature obtained, screening and visitor guidelines must be filled out and signed prior to visitation by all visitors.
  7. The resident must wear a mask or face covering at all times. The visitor must wear a mask or face covering at all times. No mask or face covering = no visitation.
  8. Maintain a distance of 6 feet between the visitor and the resident during your visit.
  9. Remain in the designated area during your visit, movement in the facility is limited to designated areas only
  10. Food is NOT permitted during visits. You may bring items for the resident, but packages must be left in the vestibule. 
  11. Visitor will NOT be able to use the facility’s restrooms during their visit.
  12. For the safety of the resident(s), Eagleview reserves the right to decrease or discontinue the visit(s) without prior notification
  13. Indoor visitations are determined by the county’s COVID-19 positivity rate (Salem and Cumberland County). If the positivity rate is<5%, then visitations may occur. However, if it goes above 5%, then indoor visits are canceled until further notice. 

Please remember that during the visitation there cannot be any hugging or touching and this may upset the resident. If this is something you believe your family member will not be able to adjust to, please do not schedule a visit with them and continue with window visits and video conferencing.

We conducted surveillance testing of all staff and residents again this week. All results are negative for the COVID-19 virus. We remain a COVID-free facility.
We have been conducting surveillance testing of all staff and residents since before Memorial Day. And we have been COVID-free among all staff and residents since May 27th. We will continue with our surveillance testing strategy in order to proactively identify any potential asymptomatic residents or staff.
We are so grateful to our staff for their tireless dedication to our residents and this home. Per the State of New Jersey Department of Health Executive Directive No. 20-017, Eagleview Healthcare and Rehabilitation will be allowing outdoor visitations. Outdoor visitations will be held outside in the front of the building from 1PM-5PM on prearranged scheduled days. Prior to your outdoor visitation, an informed consent form must be filled out by both the visitor and resident (or authorized agent). Rules for engagement include:

1. An appointment must be made prior to visitation. Please contact the receptionist at 856-451-5000 to schedule a day and time.

2. Visits may last for a 30 minute duration. Please arrive 10 minutes prior to your visit appointment time to complete pre-screening.

3. Understand that visitations could be canceled if the weather does not permit visits that day.

4. The resident (or authorized representative) and visitor must sign an informed consent form prior to visitation(s).

5. Only two visitors per visit. No visitor younger than 18 years of age.

6. Visitor temperature obtained and screening must be filled out prior to visitation by all visitors.

7. The resident must wear a mask or face covering at all times. The visitor must wear a mask or face covering at all times. No mask or face covering = no visitation.

8. Maintain a distance of 6 feet between the visitor(s) and the resident during your visit.

9. Remain in the designated area during your visit.

10. Food is NOT permitted during visits. You may bring items for the resident, but packages must be left in the vestibule. The visitor(s) may bring their own water if they want, but cannot share with the resident. Do NOT drink during the visitation (remember masks must be worn at all times).

11. Visitor(s) will NOT be able to use the facility’s restrooms during your visit.

12. For the safety of the resident(s), Eagleview reserves the right to decrease or discontinue the visit(s) without prior notification

Please remember that during the visitation there cannot be any hugging or touching and this may upset the resident. If this is something you believe your family member will not be able to adjust to, please do not schedule a visit with them and continue with window visits and video conferencing.

On Wednesday, May 27th Eagleview in collaboration with Salem Hospital conducted facility-wide testing of all residents and staff members. We are pleased to report that 100% of the results were negative for COVID-19 among all the staff and residents that we tested. We appreciate the partnership and support from our colleagues at Salem Hospital. They were an excellent team of professionals to work with. And it was a true joint effort to accomplish over 160 tests in a single day. We also want to thank our amazing staff for the good job they are doing every day to protect themselves and our residents by maintaining safe practices while away from work.

Today, we had one employee report a positive COVID-19 test result to the nursing home. This employee has been promptly restricted from duty.

In keeping in compliance with all State and Regulatory agencies, EAGLEVIEW is obliged to inform families and staff when one of our residents or staff members has tested positive for the COVID-19 virus

This past Wednesday May 20th EAGLEVIEW conducted a facility-wide testing of our residents and staff as per the NJ-DOH guidelines. We have received notice from our laboratory of three of our residents and one staff member has tested positive. We wish to advise you, the family members, that the positive residents are being isolated off in a separate area of our building and are receiving appropriate medical care under the direction of our medical director. The staff member has been removed from the schedule and will return to work after following the guidance provided by the CDC and the NJ DOH.

Normal operations have not been adversely affected and our staffing is fine to meet all required care.

As mentioned the first round of testing was completed on May 20th, a second mandated testing will be completed on Wednesday, May 27th

Resident safety is our top priority. We are doing everything we can to ensure we prevent the possibility of the spread of COVID-19 within our facility, including staying in close communication with local and state health officials to ensure we are taking all the appropriate steps.

Family members are encouraged to connect with their loved ones through video chat, calling, texting, or social media.

Eagleview Health & Rehabilitation currently has zero (0) active cases of COVID-19 at this time. We will promptly provide further updates if any new cases develop. Please know we are actively following all local and state health department, CMS, and CDC guidelines to ensure the protection of your family members.
